Diet after a gastric sleeve
The three food stages, the habits that make a sleeve work, and the vitamins you'll take for life.
The HSE describes a typical plan: liquids, including protein-rich drinks, for the first two days; soft, low-fibre, high-protein food such as yogurt, scrambled egg, minced meat and milk up to week twelve; then a gradual return to a healthy, balanced diet with fibre, fruit and vegetables. Portions stay small for good, and you take vitamins for life. Your dietitian's plan always comes first.

The stages
- Stage 1: liquids (days 1 and 2)Sips of water, then clear fluids and protein-rich drinks, a little at a time all day.
- Stage 2: soft, high-protein food (weeks 1 to 12)Soft, low-fibre food you could mash with a fork: yogurt, scrambled egg, minced meat, milk, flaked fish, soup. A few spoonfuls is a meal at first. Some teams split this into a purée stage and a soft stage.
- Stage 3: a balanced diet (from week 12)A gradual return to normal food with fibre, fruit and vegetables, in small portions, protein first.
Stages from the HSE. Teams differ on the exact weeks, so follow the plan you're given.
The habits that make it work
- Protein first at every meal, to protect your muscle while you lose weight.
- Small portions, eaten slowly. Stop at the first sign of fullness.
- Drink between meals, not with them. Many teams advise a gap either side of eating, so you don't fill the sleeve with liquid.
- Skip fizzy drinks. The gas is uncomfortable in a small stomach. The HSE suggests cutting them down before surgery.
- Go easy on alcohol. It hits harder and faster after surgery, and it's empty calories.
- Sweet and fatty foods slow your progress, and after a bypass they can cause dumping.
Vitamins and blood tests
Everyone takes vitamin and mineral supplements after a sleeve, for life. The HSE says follow-up includes blood tests to check your levels. Signs of low levels include tiredness, breathlessness, palpitations, pale skin, pins and needles and weakness: tell your team or GP if you notice them.
Questions people ask
When can I eat normally after a gastric sleeve?
From about week twelve you gradually return to a healthy, balanced diet, in much smaller portions than before.
Can I drink alcohol after a gastric sleeve?
Ask your team. Many advise avoiding it early on and limiting it after, because its effects are stronger and faster.
Do I need vitamins forever?
Yes. The HSE says to take vitamins for the rest of your life as prescribed, with blood tests to check your levels.
